Care managers are nurses and social workers who can help you better understand and manage your health conditions. They are advocates who can coordinate care with your providers and help you achieve your best health.
You, your care manager and your health care providers work together by:
- Assessing your wellness and your personal needs
- Coordinating health care services and supplies
- Building your skills so you can better manage your conditions
- Assisting with referrals to specialty providers

Get extra support when and where
you need it most:
Preventative Health
We can help you stay on track with your preventive care, ensuring you are up to date with timely health screenings, annual well checks and immunizations. We may reach out to connect you with a care manager who can guide you through a new diagnosis, coordinate care with your providers and help you to get the most of your plan.
Transitional Care
After a hospitalization, injury or serious life event, we can help you to navigate your care, get appropriate provider follow-up care and access medications and other supplies. We provide transitional support to help you to better manage your health and reduce the need to go to the hospital.
Complex Care
Our team works with you one-on-one to coordinate your care and make it easier for you to focus on your health. We can connect you with a registered nurse or licensed clinical social worker who can educate you about a new diagnosis, offer guidance to better manage a complex or chronic health condition and help you achieve your best health.
EHP Benefit
No Copay on Asthma or Diabetes Medication
Medication costs can add up, especially if you take medicines daily. If you have asthma or diabetes, you can take advantage of EHP’s No-Pay Copay benefit*. Participate in Care Management and EHP will waive the copay for most asthma and diabetes medications.
That’s right — you’ll pay nothing for certain prescriptions for as long as you are a member.** Along with the No-Pay Copay benefit, you’ll work one-on-one with a care manager to:
- Evaluate your wellness and personal needs
- Coordinate your health care
- Improve self-management of your condition

Mental Health Support
When you have a mental health or substance misuse concern, you can get confidential support from a licensed clinical social worker who can:
- Teach you about your mental well-being
- Discuss your treatment needs
- Help you connect to a mental health or substance misuse specialist
- Coordinate your care between providers
- Offer resources to help you understand and manage your medications
